Does Training Intensity Outperform Moderate Cardio for Fitness Gains?

An eight-week randomized trial in sedentary young men, published in BMC Sports Science, Medicine and Rehabilitation, put four protocols head-to-head: HIIT, high-intensity continuous training, moderate-intensity interval training, and moderate-intensity continuous training. HIIT raised VO2peak more than moderate-intensity interval training. High-intensity work — both interval and continuous formats — also outperformed moderate-intensity protocols on selected blood-pressure and rate-pressure-product measures. Body-composition outcomes did not differ between groups.

Where Intensity Earned Its Keep

The cardiorespiratory data is the load-bearing finding. Higher-intensity protocols produced a larger VO2peak gain than moderate-intensity interval training within the same eight-week window. Rate-pressure product — a product of heart rate and systolic blood pressure that indexes cardiac workload — and certain blood-pressure markers also tilted toward the high-intensity arms. For sedentary men beginning a conditioning block, the conclusion is direct: if VO2max is the metric that matters, intensity separates the protocols. Format — interval versus continuous — mattered less than the absolute effort level. Both high-intensity arms beat at least one moderate-intensity comparator, which is the more useful takeaway than any single head-to-head.

Where the Data Was Quiet

Body composition told a different story. Fat-mass and lean-mass changes did not differentiate between HIIT, high-intensity continuous, moderate-intensity interval, or moderate-intensity continuous training across the trial. The training stimulus moved cardio fitness but did not remodel tissue composition in this window. Anyone chasing visible recomposition through cardio modality alone is reading the wrong variable. Caloric deficit, protein intake, and resistance training remain the actual drivers of body-comp change. Eight weeks is also short for that signal to surface — body-composition deltas typically need longer timelines and tighter dietary control to register reliably, particularly in untrained subjects with limited starting lean mass.

The Verdict

For VO2max: do high-intensity work. Three to four sessions per week, twenty to thirty minutes each, is a defensible structure for an untrained male starting from sedentary. For body composition: skip the modality debate. Pick a calorie and protein target, add a resistance routine, and use the cardio session as a conditioning finisher — not as the primary lever. The cardio piece supports the system. It does not carry it.
